How to Change a Birth Certificate in Colorado

If you are 18 years of age or older you may apply to change your birth certificate in Colorado. If you are a minor, the application must be made by your parents or legal guardian. All changes to a birth certificate must be accompanied by the Form to Correct or Change a Colorado Birth Certificate, the incorrect item as listed on the birth certificate and the corrected item as it should appear. Additional documentation may be required for some things including paternity determination and gender change.
Obtain the Form to Correct or Change a Colorado Birth Certificate form that may be downloaded from the Colorado Department of Public Health and Environment website or obtained directly by calling 303-692-2200 or by mailing to:

Fill it out completely using ink. No whited out or crossed out items are permissible.

Make copies for the county of required documentation, including one or more documents that support the facts pertaining to the reason for the change on your birth certificate. These documents must have been created no more than five years prior to your application or, in the case of a child, within seven years of the date of birth. The documents must include your date of birth. If that is the only information on the document you must provide two pieces of documentation unless you are only changing the spelling of your name. Include a check, money order or credit card information (there is a $9, as of 2010, convenience charge for credit cards, walk-ins not included) for the correct amount listed on the form. The office will accept cash if you are taking it to the office, but is not responsible for cash lost in the mail. The cost for changing a birth certificate is $20, as of 2010, but this does not include a copy of the birth certificate, if you want a copy it will cost, as of 2010, an additional $17.75 for the first copy and $10.00 for each additional copy.
